Integrated Health Outcomes Coordinator – This position has the responsibility of overseeing the daily operations of care coordination program and providing supervision to assigned staff. This includes (but is not limited to) staff acquisition, staff development, acting as a liaison between the agency and state and/or Managed Care Organizations and assisting the program director. Bachelor’s Degree or equivalent combination of education and experience in the Human Services Field required.

Starting wage is $33,120.00 per year.
