Hospital Liaison-Patient Navigator – This position entails facilitating partnerships with state and other private acute psychiatric hospitals through participation in routine hospital meetings, linking patients with resources to overcome identified barriers to treatment, coordinating appropriate treatment and assisting with patient crisis episodes as needed. The selected candidate must be an outcome focused individual that seeks to help clients have an effective and efficient experience while receiving care. This position is geared towards increasing access to and improving the quality of community mental health in pursuit of healthier communities.

A Bachelor’s degree or equivalent combination of education and/or experience in the human services field is required.

Hourly position: starting wage is $40,000/year – $19.23/hour.
