How To Request Services
      To inquire about services available in any community or to request services, you may phone the Independence office, (620) 331-1748, which is open from 8:00 AM to 6:00 PM Monday, Wednesday, Thursday and Friday and from 8:00 a.m. to 8:30 p.m. on Tuesday.
     If the inquiry or request is for Coffeyville you may phone the  branch office in Coffeyville, (316) 251-8180, which is open from 8:00 a.m. to 6:00 p.m. Mon­day, Tuesday, Wednesday and Friday and 8:00 a.m. to 8:30 p.m. on Thursday.

Who is Eligible for Services
     Anyone is eligible to receive services through Four County.  However, a charge over and above the fee arrived at for outpatient services is charged to individuals who reside outside the four counties served.  Specialized programs at Four County may have defined criteria for acceptance into the program. 
Charges for Services
     Private fees are set by the Board of Directors.  Clients may be eligible for a discounted fee based on the family's income and the number of family members dependent on that income.  Medicare, Medicaid and most health insurances are accepted.
Employee Assistance Programs
  
This program provides consultation, counseling and evaluation services to area employers.   This program includes mental health, alcohol/drug counseling and evaluation services for employees of a contracting business or industry, and consultation services to the business or industry.
