Senior Outreach Services, or SOS, provides
specialized services to Montgomery County residents and provides
confidential in-home mental health or substance abuse assessments.
Case Management, collaboration with the family physician, counseling,
education, screenings, and referrals to appropriate community services will
be made available. This program is an added component to the services
already offered by Four County to the senior population. |

How To Request Services
Services may be requested by contacting
either our Independence (620) 331-1748 or Coffeyville (620) 251-8180) office.
We also accept referrals from physician’s, nurses, physical therapists,
ministers, and family members. |

Who is Eligible for Services
The Senior Outreach Program is designed for individuals 60 years of age or
older who are living at home or in senior housing facilities, including
assisted living communities. |

Mental health problems are not
part of the normal aging process, but many of the circumstances facing older
adults can contribute to the development of depression and other mood
disorders, anxiety disorders, or substance abuse. If someone has
experienced a recent loss, has noticed a change in ability to manage daily
routines, has recently had a loss of independence, is sneaking drinks, has
flashes of aggression, has unreasonable resentments, has had a change in
sleep habits, feels anxious or depressed, feels like life is no longer worth
living then the services may provide the needed help for a more fulfilling
life. |
