Liaison Services
   Four County Mental Health Center's Liaison case manager works closely with Osawatomie State Hospital staff when individuals are admitted to this sate facility. The liaison is at the hospital weekly participating in treatment and discharge planning. The liaison works as a mediator between the two agencies; relaying relevant information, taking part in case conferences, assisting with discharge planning and the coordination of follow-up outpatient services for people who have to visit the hospital, making their transition home more convenient and timely. Liaison's are also available to local private hospitals.
