     SHARED LIVING   

CRITERIA

     The purpose of Shared Living is to provide a safe, affordable environment where patients can have housing, learn independent living skills, have assistance with medication compliance, be assessed for needs with varied levels of services per their individualized Treatment Plan that is approved by our Medical Director. Each person works directly from their Treatment Plan and will receive special assistance regarding the goals by Community Support Services Case Management and 24-hour support from our Community Support Specialists.
     Shared Living is simply a living environment where space is shared and where an array of community support services can be provided. Each residence is completely furnished with cooking supplies, furniture, linens and appliances. While at Shared Living each patient is responsible for minimal rent, food, medications and personal items. With rent being low cost, this is a great opportunity for a person to save money that can be used to pay deposits and items for their own residence once they are ready to move into independent living.
  • Adults 18 and over
  • Coming out of a more restrictive environment or at risk for entering a more restrictive environment
  • Willingness to sign and comply with the Shared Living agreement
  • Acceptance of referral by the administrative team of Shared Living
  • An assessment of level of services/supervision/ support needed by the patient
  • Specific treatment plan, based on the assessment which established the need of the individual
  • Has a reliable method of paying rent
  • Has had or been seen by a medical doctor in the last 12 months or is willing to see a doctor
 TEMPORARY HOUSING

      The focus of Temporary Housing is to be a housing resource for enabling adult patients to get back in the community after being hospitalized, homeless, or a lapse in service due to a crisis in daily living situations.   
     The purpose of this program is to provide and utilize Temporary Housing for patients who are Severely and Persistently Mentally Ill and in a housing crisis. Temporary Housing provides a secure living environment for patients to locate housing and re-establish services in the community when coming out of the hospital, institution or is left without any financial support or shelter.
