Case Management services are provided to adults who
experience severe and persistent mental illnesses. Successful case
management is accomplished by developing a trusting relationship with
patients, empowering them to work towards recovery, independency and
rekindling hope. Components included in case management might be: |

- Assessing the patient’s needs and
strengths
- With the patient, develop goals for
their life and treatment
- Networking with agencies both
internally and externally to provide necessary services
- Monitoring mental heath symptoms
- Medication education
- Focusing on day to day concerns of
living in a natural setting

Intensive Case Management |
- When a patient returns to the
community from hospitalization, nursing home or group home placement, case
management services can be intensified.
- Other community services, such as
attendant care, outpatient therapy, community integration, etc can be
“wrapped around” the patient to ensure their successful reintegration into
the community

Extended Hours Case
Management |

|
- Extended Hours Case Management is
provided to assist patients outside the traditional office hours.
- Evenings and Weekends can be
difficult times and we can assist patients in developing coping skills and
learning to use them during these timeframes.
