MOTHERS ON THE MOVE SERVICES
(Teenage Pregnancy Case Management)

     The MOMs Program/Teenage Pregnancy Case Management is sponsored by a grant from the state of Kansas, Department of Health and Environment, and is designed to provide community based services for pregnant and single mothers so that they may continue to pursue their educations, whether if by High School, vocational, or GED. The overall goal of the program being to promote long-term self-sufficiency and independence. The program targets pregnant females under the age of 21 or a mother under the age of 21 with one or two children. The mothers must be Medicaid/Healthwave eligible, and must be residents of Montgomery County.  There are no fees for participating on the program, but the mothers must meet the above-stated requirements.  The Teenage Pregnancy Case Manager functions as liaison between the public school system and the mother, coordinates medical and health care assistance for the mother and child/children, facilitates development of a long-term self-sufficiency plan, facilitates and shares information on parenting skills for the mother, offers one-on-one contact, and coordinates evaluations and assessment of the infant/child’s development.
