“Foundations”
is an intensive transitional skills program with the purpose of providing
youth and young adults with Severe Emotional Disturbances (SED) the
necessary tools and guidance for successfully transitioning into
interdependent life as an adult. “Interdependent” is used vs. “independent”
as the philosophy of the program focuses on the belief that as humans, we
function at our best when we are connected with others in the community in
which we live. While teaching skills that will focus on independent living,
there will always be an emphasis on the need for interpersonal relationships
with other members of the community such as friends, co-workers, civic
groups, religious organizations, etc. |

Guidelines |

The goal of Foundations is to provide a medically necessary
therapeutic environment in which patients can actively participate in
learning activities of various kinds to gain the necessary skills for
independent living and a hope and belief that one can make it on his/her
own. |
- Must be Age
14-21
- Currently
living in Chautauqua, Elk, Montgomery or Wilson
Counties.
- Are currently
diagnosed with a Severe & Emotional Disturbance (SED)
- The services
are deemed medically necessary by the patient’s treatment team
- Engaged in
Community Based Services through Four
County Mental Health Center.
- Completion of
an Ansell Casey Life Skills Assessment with their case manager
- Submission of
referral packet from Case Manager

We work to accomplish this goal by developing learning activities that focus
in on ten different life domains. They are Career Planning, Communication,
Social Relationships, Daily Living, Home Life, Self Care & Wellness,
Work/Study Skills, Work Life, and Housing/Money Management issues.
Learning activities will occur in various community based settings and will
allow patients to have hands on experience in real life situations while
being supervised in both group and individual settings. |
