If you would like to order Educational Cards please complete this form.  These topic cards may be printed by you or a commercial printer.  Four County has made available these cards at a minimal price of $0.20 per card (shipping included).  If you would like for us to customize your logo and information on the back of each card we can do so.

PAYMENT DUE AT TIME OF ORDER
(We are not able to accept credit cards on line; however, you may call in a credit card number to Susan Hill (620) 331-1748 or send a check - to Four County Mental Health Center, PO Box 688, Independence, KS  67301 Attn:  Susan Hill).  Once payment is received cards will be shipped.  Postage is included in the cost of the cards.
